Install the frame bracket with one part inside the frame and the other part outside the frame like this.

I'm sharing some difficulties and solutions that I had in installation.  First as the instructions described the buildup of powder coat in the bumper bracket pivot for the tire carrier did not allow the pivot shaft to go through.  I ended up wrapping some sandpaper around a socket extension to remove the material semi evenly.  This really seems like the same tool used to create the holes at the factory could be used to clean out the holes after powder coat so it was a little frustrating to have to do it myself.  Also later i found out that the shaft wouldn't go through the two bearings (you could definitely tell that the shaft was a bit distorted in the middle causing the interference) so I ended up using a small belt sander to remove enough material around the shaft to allow it to get through.  This is something else that created some frustration. On the consumer end of this we really should be Assemblers not Fabricators. 
Overall I'm really happy with the rear bumper.  It took a bit of customization but ended up very nice in the end.
